A community health improvement plan is a long-term plan, describing how the local health department and a broad set of community partners are addressing needs identified in the last community health assessment. This 2022-25 plan is based on a community health assessment completed for Washoe County in 2022. The Community Health Assessment Steering Committee reviewed the top health issues from the 2022 assessment and identified four underlying health priorities for 2022-25 (see below).
Many organizations are involved in addressing these priorities. Completing and monitoring the plan, in collaboration with community stakeholders and partners, is a responsibility of the Washoe County Health District under Nevada Statutes §145A and is required by the Public Health Accreditation Board. WCHD leads the Community Health Assessment and will compile updates and revisions to the plan in annual reports. “Community” refers to the whole population of Washoe County, as opposed to the health of any one individual. “Health” includes not only our health outcomes but also the health factors that influence health. This plan is about improving the health of the community together and achieving the Health District and community’s vision.

The CHIP is a collaborative effort with the community to address public health challenges based on the results of the 2022-25 Washoe County Community Health Assessment (CHA) activities and community inputs. We’re pleased to announce the four 2022-25 CHIP focus areas:

The CHA rankings reveal a snapshot of how health looks in Washoe County by showcasing how health is influenced by factors where we live, learn, work and play. Secondary and primary data were examined and ranked by community need in the CHA. The rankings compare the health of other counties within Nevada, and the US to provide a starting point for change. The Washoe County Health District’s Equity team also conducted a listening tour with community leaders to gain additional input to inform the CHIP planning process.
